Reasonable Accommodations Procedures

1.  Employee completes the Employee Request for an ADA Reasonable Accommodation Form and the ADA Authorization for Release of Medical Information Form and sends them to the CSN Human Resources Department.

2.   Upon receipt of the request, Human Resources meets with the employee to clarify the request and get additional information.

3.   Human Resources reviews the current position description for the essential functions. When necessary, Human Resources contacts the Supervisor for information on the job’s essential functions.

4.   Human Resources will send a completed ADA Authorization for Release of Medical Information Form to the medical provider.

5.   The medical provider completes the form and sends back to Human Resources contact; supplemental information or clarification may be required.

6.   After receiving the information from the medical provider, Human Resources will meet with the Supervisor to discuss the request.  If it is determined that the employee is ADA qualified and the accommodation request is not an undue hardship, an accommodation will be approved.   The accommodation may not always be specifically what employee requested.  The funds to cover ADA accommodations come from a special limited fund set aside and managed by Human Resources. If the requested accommodation is denied, the employee may appeal the decision (See step 7).

7.   To appeal, the employee must complete the ADA Appeal Form within 5 days of receiving the written response to the request.  The appeal goes to the General Counsel who has 10 days to make a final decision.
