Reporting Your Concerns
This web site was built as a resource for you to express your concerns about activities at CSN. You have several alternatives on how you can get attention drawn to areas of your concern.
Your concerns might include questions about activities being proper and authorized at CSN. They might include your need to report suspected improper activities to include a broad range of fraud, abuse, mismanagement, waste, misconduct, non-compliance and other wrongdoing. Anyone at CSN who has reasonable suspicions has the means for reporting their concerns.
CSN is committed to institutional integrity, accountability, and compliance with the policies, rules, regulations, and the law. CSN believes in doing the right thing. If you are aware of practices that are not consistent with these values, it is only through communication of your concerns or questions that CSN can address them.
If it is possible, we encourage individuals within the college to initially discuss concerns with their supervisors. We know that is sometimes difficult to do, especially if the supervisor may be involved in the area of concern.
In cases where discussion with your supervisor does not resolve the situation or suspected wrong-doing may involve an employee's supervisor or you are just not comfortable reporting in person, additional means are available to report the issue – See below.
Please note that disputes with supervisors, such as disagreements regarding performance evaluations or merit increases are not considered reportable issues within the context of this reporting mechanism. These concerns should be addressed through normal department channels, beginning with a conversation with your supervisor. If that does not resolve the issue, contact Human Resources.
Reporting Contacts
There are several offices on campus that have been charged with the responsibility of conducting reviews based on their expertise and areas of responsibility. Offices at the college with responsibility for specialized reviews and investigations include:

2. CSN Internal Audit
Concerns about CSN activities involving misuse and/or theft of CSN resources, financial misconduct, fraud, waste, abuse, and embezzlement should be forwarded to the CSN Internal Audit Office. Several options are available for contacting the Internal Audit Department and within those options are opportunities for anonymity:
- By Email to a “Concerns” email box: Send an email to CsnConcernsReporting@csn.edu. The two auditors in the department are the only ones who will read messages sent to this email.
- By Email to either of the CSN Internal Auditors. See addresses below.
- By Phone: See numbers below.
- By Surface Mail: See addresses below.
- By Reporting Form. See the CSN Concerns Reporting Form which can be submitted anonymously by surface mail.

3. Equal Opportunity and Affirmative Action Issues
This reporting system will forward concerns and complaints that involve sexual harassment, affirmative action, and equal employment opportunity claims to appropriate CSN offices. If claims are received that involve sexual harassment or related issues, they will be forwarded to Human Resources at 651-5800. Complaints that involve discrimination or affirmative action issues will be forwarded to Thomas Brown at 651-4002 (CSN Affirmative Action Officer).
