• New Hire Packet (contains all of the forms below in one PDF file.  Please note that the Personal Data Form, I-9, and Employment Application files are contained as attachments within the PDF.  If you cannot open them, please download them below.  Also, please note that the FICA Alternative form is no longer required)

• NSHE / CSN Policy Statements Receipt Sign-off
• Current and/or Change in Family Relationship
• Exposure to Bloodborne Pathogens Determination

• NSHE Sexual Harassment Policy Acknowledgement
