Verification Forms
2023 - 24 (Spring 2024 & Summer 2024)
If the U.S. Department of Education has selected you for verification (group 1 or group 5), you would have received an email or a postcard from our partner Inceptia providing instructions.
Students selected in verification group 4 must complete the item listed below.
- 2023-2024 Statement of Educational Purpose
*If submitted through post, this form must be notarized and stamped by an in-person notary service (online notaries are not accepted). In addition, a copy of an unexpired government photo ID must be submitted with these forms.
2024 - 25 (Fall 2024 - Summer 2025)
If the U.S. Department of Education has selected you for verification (group 1 or group 5), you will be emailed by our Partner ProEd to complete verification using ProVerifier+
Students selected in verification group 4 or group 5 will be required to submit the Statement of Educational Purpose in-person or via post. If submitted through post, this form must be notarized and stamped by an in-person notary service (online notaries are not accepted). In addition, a copy of an unexpired government photo ID must be submitted with these forms.

We highly encourage students to consult with the Office of Financial Aid before submitting any of the appeals below. Incomplete appeals are subject to denial. Please read all directions carefully and submit all documents in one complete submission. Please follow the instructions below.
