Promise Financial Aid Forms
The College of Southern Nevada welcomes all students. As a Nevada Promise Scholarship applicant/recipient, you may need to supply additional information due to eligibility requirements or deadlines. The following forms are provided to assist you.
CSN cannot waive or change the statutory requirements set by the Nevada Legislature, however, if you believe you met the requirements or have extenuating circumstances, please complete the form that best fits your situation. Please note these forms need to be returned to the Financial Aid Office at one of following CSN campuses:
