Accessibility - Documents - Correcting the "Title - Failed" issue

Back to Wiki

This step-by-step illustrates how to correct the missing title issue in a PDF or Microsoft Document and assumes you have already run the accessibility check in Adobe Acrobat Pro 2017. Any screenshots shown will display the view in Acrobat Pro 2017 and Microsoft Word 2016. Most, if not all, Microsoft programs will be the same steps as Microsoft Word.

Correcting the Title issue in an Adobe Acrobat PDF

  1. If you have a source document for this pdf, follow the set of instructions below in Correcting the Title issue in a Microsoft Document to correct the issue in that file. Otherwise, you will be correcting this issue everytime to update the file then export/print to pdf.
  2. If you only have the pdf and need to edit it, ensure the file is opened in Adobe Acrobat.
  3. Select "File" > "Properties" and a popup dialog appears.
  4. In the "Title" field, ensure that there is a readable title without any non-standard abbreviations. For example, "CSN" is acceptable as everyone knows what this is. "AIP" is not acceptable as most folks may not know what it means OR could be an abbreviation for multiple names. Using fully readable words helps screen readers clarify to their users what this information is about.
Important: It has been determined that Adobe Acrobat may show this failed error even when the title is present. If this is the case, just follow the remaining steps.
  5. Once the title is entered, select "OK".
  6. Now, right-click on the  "Title - Failed" error and select "Fix". The error should go away and a green checkmark should appear next to it.
  7. Save the PDF.

  3. If there is no text here, you will see placeholder text "Add a Title". Click here and add the title of the document.

  4. Save the Microsoft  Document.
Important: When you print/export to PDF, check the pdf with Adobe Acrobat for accessibility issues. Sometimes the title issue will appear in Acrobat when running the accessibility checker, even though you have added it to the file.
