Students can be selected for Identity Verification either by CSN or by the Department of Education. Please review your To Do list for the type of ID verification you must complete.
In-Person Option
If your MyCSN To-Do List requests verification of ID, please visit your nearest campus and bring your unexpired, government-issued ID.
Other Option(s) - Requires documents to be NOTARIZED (no online notaries are accepted)
- If CSN selects you for ID review and you cannot come in person, you may submit the CSN’s ID Verification form by mail or email. You must include a copy of your valid, unexpired government-issued photo ID (FRONT and BACK copies of ID is required).
- If you are selected by the Department of Education for verification (V4 or V5) and cannot appear in person, your only option is to submit the form by mail, note email is not allowed. You must include a copy of your valid, unexpired government-issued photo ID.
Be sure to mail all required documents to the address listed on the form. Incomplete submissions will not be accepted.

Fall 2026 SAP Appeals (Available July 3, 2026 - September 22, 2026)
Satisfactory Academic Progress Appeals can require extensive effort and documentation.
Things to consider
- Semester Payment Deadlines are three weeks before the term begins. The Office of Financial Aid does not hold classes for students in the process of an appeal review. Please discuss payment arrangements with the Cashier's Office.
- Students cannot submit a SAP Appeal if they are not suspended. Our Office evaluates Satisfactory Academic Progress 8-10 days after a term ends.
Fall 2026
Last day to submit a Fall 2026 SAP Appeal is September 22, 2026.
We highly encourage students to consult with the Office of Financial Aid before submitting any appeal. Incomplete appeals are subject to denial. Please read all directions carefully and submit all documents in one complete submission.

Please make sure you are enrolled in the semester for which you are requesting a loan and registered in at least 6 financial aid-eligible credits that apply toward your declared major. Students enrolled in 12 or more eligible credits may qualify for the maximum loan amount. Students enrolled in 6–11 eligible credits may still apply, but the loan amount may be reduced based on enrollment.
Also, please be sure that your 2026-2027 Financial Aid Checklist Items (aka To-Do list on MyCSN) are complete before submitting the loan request form. If you have not completed your Master Promissory Note (MPN) and Entrance Counseling, you will need to do so at studentaid.gov in order for your loans to disburse.
