Fully complete the following documents to include securing approval signatures required.

1. Reclassification Request Guidelines
2. State of Nevada Position Questionnaire (HR-19) to include supporting documents
3. Personnel Transaction Form
4. Resume of Incumbent
5. Submit all above documents via email to HR.Recruitment@csn.edu

Professional – Administrative Faculty

Fully complete the following documents to include securing approval signatures required.

1. Professional Position Reclassification Form
2. Personnel Transaction Form
3. Resume of Incumbent
4. Current Organizational Chart
5. Proposed Organizational Chart
6. Current Job Description of Position to Requested to Reclass
7. Proposed Job Description (highlight in yellow or use MS Track change feature to identify new duties)
8. Provide a list all NEW responsibilities or duties being performed, identify position(s) also currently performing responsibilities or duties and/or performed them in the past.
9. Submit all above documents via email to HR.Recruitment@csn.edu
